I guess any shop, real or web would do - I currently buy size S or XS for clothes and European size 35 / 36 for the shoes (36 for boots and 35 for peep toes) - but have no doll yet as its still on order.

I don't know how this post came up high on the list for me to notice it . . . but the two questions are helpful

Size - S is fine although she's a bit narrower at the shoulders.

Shoes - For 158 size 38 and for 168 size 35 but actually 34 would be better. I have bought 35 and they slip off a little too easily and I'll have to pad them slightly.

S is sometimes a little to large, so I tend to buy xs or 34 (German), as well I purchased some kid size 152 and 158 for my 168+ - which are fine for tops (152 is too small for skirts! unless you like them very tight or they have enough lycra in them, no experience with 158 skirts yet - thats on the list). Women size 32 (German) is as well on the tight side for skirts or shorts.

Shoe size is on the smaller end of 35 - so haremlover is right, 34 works better on some shoes, ie peep toes.
But this is as well manufacturer dependent - I have size 35's that fit perfect and others that are slightly too large and need some padding.

Boots I will continue to using size 36 - the theory behind as follows (without guarantee my thinking is correct!!)
- less or no weight on toes when standing
- in heels the feet are not bend as much, as the do not fill the complete shoe
- therefore less risk that the foot plate breakes through the feet, damaging the doll
- not visible from outside that they are too large

I always have her wearing socks or overknees in boots
