I have been using a heating rod that maintains temperature at 45c without any issues. Less than $10 and way better than the cheap ones that are sent with the dolls.

Sounds like you have a lot of things to try.

One thing to consider for the insert... baby powder that is based on talc will not feed mold and mildew like cornstarch will. I know people don't like the talc due to health concerns but I still prefer it over cornstarch.

Really sounds like surface abrasion to me. I had something similar near the arm pits and fixed it by applying extremely light heat from my heat gun, just enough to get the surface to flatten again. While I did this using a nice coating of mineral oil, I didn't do it enough to require re-texturing us...

Given the current 'Doll Climate', creating monster-girl dolls will be a good way to avoid some of the issues.
