I think there's a limit on how close the two holes can be on a doll.
If they were anatomically perfect there would be too little material between which would tear much too easily.

I think the vagina location should govern. If the anus is a little higher than normal, so be it.

Not really.
With her butt stuck out and back arched like in the photo, the vagina would be clearly visible from behind, and with the vaginal entrance correctly located at the back it can be penetrated in the standing position.
I know we're not supposed to show human genitalia, but this is strictly for illustrative purposes...

I am very happy that 6YE address also this problem. The sex organs of my 165F for example are very similar to nature and of good volume. THe only think they need is a correct positionning.
It can be solved easily provided the sculptors copy exactly the sex organs and anal holes and their position as they can observe them on well made real women without compromise.
When one doll will show a back like the mannequinfan attachment then I will congratulate the sculptor and immediately buy the doll (if other realistic body ratios are respected of course as I am a realistic dreambody ratios fan)
