M5 is the "Thomas" head as I like to think of it and the M3 is the "George" Head (although I've seen dolls with this head on some sites also named "Enzo"). They just look different, no differences.

You can order directly from the manufacture or a vendor as well, your choice, I am yet to order my first doll so I can't really tell you what is "better" it might just be personal preference and maybe what vendors you have available to you. I have been told though that it can be cheaper to order directly from the manufacturer, I'm not sure of course... it's just what I've been told.

We have ordered both ways. We saved a ton ordering directly from Irontech. Winnie helped us! She was great!!

Just like the female dolls they are 2 different heads. You can pick skin tone from a few colors, implanted hair or wig from several styles/colors, eye color etc... they can also have many looks just from changing things on one. Just depends on what you like.
